Simulation and Virtual Reality in Medical Education: What Actually Improves

Virtual learning environments and virtual reality (VR) simulation have stopped being a technological novelty in medical education and become an instructional design option with evidence of its own. A meta-analysis evaluating VR-based medical education programs, after selecting 25 studies through systematic review and 18 for the final meta-analysis, identified a significant improvement in skill level and satisfaction for the group using VR compared to conventional methods [1].

That same analysis offers an important nuance for anyone designing these environments: less immersive VR proved more effective than fully immersive VR for knowledge outcomes, even though both improved skills and satisfaction [1]. It is a counterintuitive finding, since more immersion is often assumed to automatically mean more learning, when the evidence suggests that the pedagogical design of the scenario matters more than the level of technological immersion itself.

A systematic review focused specifically on anatomy education, published in Cureus, evaluated the effectiveness of VR simulation in improving medical students' learning outcomes compared to conventional teaching methods, such as cadaveric dissection and anatomical models [2]. The study starts from a real problem with traditional methods: their limited accessibility and the difficulty of visualizing complex three-dimensional structures — something immersive environments address directly [2].

For a team evaluating whether to invest in a virtual learning environment or VR simulation, the practical conclusion from the evidence is clear: the technology offers a real advantage, particularly when the content requires spatial visualization or repeated practice without risk — anatomy, procedures, emergency scenarios — but that advantage depends on the scenario being designed with clear learning objectives, not on the sophistication of the hardware used.
