Gamification in Medical Education: Fad or Evidence-Based Method?
Gamification — the use of game elements in non-game contexts, such as points, badges, challenges, or leaderboards — has become one of the most widely used and least understood methodologies in medical education. The question worth asking before adopting it isn't whether it's popular, but what the evidence says about its real effectiveness.
A systematic review published in Advances in Health Sciences Education analyzed 44 empirical studies on gamification in health professions education, after screening more than 5,000 initial articles. A notable finding: no included study reported negative outcomes from the use of gamification [1]. Most studies combined assessment elements (40 of 44) with conflict or challenge elements (27 of 44), and eight of them showed that this specific combination increased use of the learning material, in some cases with measurable improvements in learning outcomes [1].
A more recent systematic review, focused on clinical competency development between 2020 and 2025, found improvements in clinical reasoning, technical performance, communication, and self-regulation associated with the use of gamification, although the authors themselves note recurring methodological problems and curricular integration challenges in the available literature [2].
The common thread between both reviews matters: gamification doesn't work simply because it includes playful elements — it works when those elements are designed with clear pedagogical logic — calibrated challenge, timely feedback, meaningful progression — and not as a decorative layer over content that, underneath, is still a lecture with points attached.
For a medical or continuing-education team, this translates into a concrete design criterion: before gamifying a training session, it's worth asking which specific game mechanic is solving which specific learning problem. If the answer isn't clear, the gamification probably won't be either.
